Event details

Keep photos, exchanged driver details, location notes, and a short timeline in your own words.

Police and report details

Keep the agency name, officer details, report number, and the downloaded report. Use Ohio Crash Retrieval(opens in a new tab).

Vehicle details

Keep the tow yard receipt, repair estimate, rental details, property photos, and other receipts.

Insurance details

Keep the claim number, adjuster name, call dates, letters, declarations page, and any requested forms.

Health-care documents

Keep discharge instructions, provider details, bills, records you receive, medication instructions, and appointment dates. Store sensitive records in a safe place.

Work and expense notes

Keep employer notes, missed-time records, mileage or transportation receipts, and other real expenses. Do not guess a loss or round numbers from memory.

Use clear filenames

Start files with YYYY-MM-DD, then a short description: 2026-07-28-portsmouth-police-report.pdf. The date sorts the story without a complicated system.

Keep private information out of website forms

Do not send this folder through the clinic’s public callback form. Ask each recipient for its approved secure channel.
