• Specific records or date range requested.
• Preferred secure delivery method.
• Firm contact and billing instructions.
• Relevant deadline stated realistically.

Do not send sensitive information through the public appointment form.

What the record contains

Records may include:

The record should not be written as legal advocacy or accident reconstruction.
