Narratives, forms, and testimony requests

Call about the office's policy for narratives, forms, testimony, and conferences — including scope, fees, and timing. This page does not promise a turnaround time.

What the office does not provide

The office can explain its records and respond to authorized health-record requests. It does not decide fault or figure damages. It does not read policy language for the patient or give legal advice.
