Hand, wrist, and arm pain after bracing for impact
Published by Portsmouth Accident & Injury Center.
Gripping the steering wheel, reaching toward the dashboard, or tensing before impact can be followed by symptoms anywhere from the fingers to the shoulder. "I braced" does not identify the injury. Separate each area, note the grip and movement changes, and seek prompt care for deformity, severe swelling, a cold or discoloured hand, major weakness, or numbness that is getting worse.
The steering wheel keeps the hands in one place, but the force does not necessarily stay there.
A sore thumb may share the story with a stiff wrist. The elbow may feel normal until you lift a mug. The shoulder may tighten later because the whole arm was held rigid.
Do not compress that chain into "arm pain." Give each segment a sentence.
How can bracing affect more than the wrist?
When a person anticipates contact, the hands may grip, the elbows may lock or bend, and the shoulders may tense. A hand can also strike the wheel, dash, door, or airbag area(opens in a new tab). The exact movement is often hard to remember.
Tell the evaluator what you do remember, without filling gaps:
- One or both hands on the wheel
- A remembered contact point
- Airbag deployment
- Thumb position if known
- Arm position immediately after the crash
- The first task that revealed the symptom
"I think my arm was straight, but I am not sure" is a perfectly acceptable history.

What should you test—and what should you leave alone?
Notice normal functions: holding a phone, turning a key, buttoning a shirt, opening a door, carrying a light cup, or typing. Do not repeatedly squeeze a grip device, do push-ups, or pull on a painful finger to see whether it is injured.
Worth writing down:
- Exact location and side
- Swelling, bruising, or visible change
- Numbness or tingling, by finger
- Grip change or dropped objects
- Motion that is limited
- Whether symptoms travel from the neck or shoulder
Remove rings promptly if a finger or hand(opens in a new tab) begins swelling, unless doing so would cause injury. Seek medical help when a ring is trapped on a swollen, painful, or discoloured finger.
Which arm or hand symptoms need urgent care?
Do not assume tingling is "just a pinched nerve." Location, onset, neck symptoms, circulation, and strength all matter, and wrist symptoms in particular(opens in a new tab) have more than one possible source.
Could the symptom start in the neck or shoulder?
It could. Nerves and muscles connect the neck, shoulder, arm, and hand. Symptoms that travel, change with neck position, or involve several regions may prompt a broader examination.
They may also represent separate direct injuries. The arm and hand symptom page and shoulder symptom page explain what a non-emergency musculoskeletal evaluation can consider.
What should a reassessment compare?
Use functions that matter:
- Gripping a normal object without dropping it
- Typing or using a mouse for a set interval
- Turning a key or opening a jar
- Reaching to a shelf
- Putting on a jacket
- Safely controlling the steering wheel
A symptom may feel similar while function improves, or pain may ease while weakness persists. Both pieces belong in the progress note.
